Dog or Puppy Bill of Sale Form

Dog or puppy bill of sale form is the bill used when selling and purchasing a dog or a puppy. This is a general bill of sale which is used in all States of USA except where a State uses its own sale form. Every State is having its own laws regarding the dog or puppy bill of sale form and the transaction will be governed by the laws of that State.

Dog or puppy bill of sale form is a kind of receipt and is a proof of making the payment to the seller. This bill of sale form is a warranty for the safety and security of the buyer, the seller and the dog or the puppy.

Filling Of the Dog or Puppy Bill of Sale Form

Section 1

At the beginning of the bill, there are two columns, one for the seller and the other for the buyer. In this section, the seller will give his/her name, address, phone number and e-mail address. In the same way, the buyer will also print his/her name, address, phone number and e-mail address. The printed name means the name of the official documents of the seller and buyer. Official document means citizenship card and passport.

At the end of this section, there is a statement, which states that the dog or puppy has been handed over to the buyer for a certain amount, which is mentioned in the space meant for this purpose. Along with the physical ownership, legal ownership of the dog or puppy has also been transferred to the buyer. The specifications of the dog or puppy are given in the next section.

Section 2

In this section, the name of the dog is to be given, the breed to which the dog or puppy belongs, color (white, black or mixed), and sex (male or female) and date of birth is to be mentioned. If there is any registration, the registration number is to be given. Sire name means the name of the father and dam name means the name of the mother. Father’s registration if there is any is to be given and the mother’s registration if she is having a registration. In the next column, “Bred by Seller”, means if the dog or puppy was bred by himself/herself and if the answer is “no”, then the name of the breeder is to be given in the next column. In this case, the date of acquisition of dog or puppy is to be given in the next column. State of health of the dog or puppy is to be given in detail.

Section 3

In this section signature of the seller is required and also the date in the next column. Signature of the buyer is also required in the next column and the date in the next column.
